Office Policies

Your appointment

We run an efficient, on-time office, and it is essential that all necessary paper work be completed prior to your appointment time.  We will typically mail our patient registration and medical information forms to your home for you to complete prior to your arrival.  Please bring your insurance card and complete list of your current medications to your appointment.

Financial Policy

Co-payments and deductibles are expected at the time of service.  We will call your insurance company prior to your visit to obtain benefits and eligibility information, so as to provide you an accurate estimate of the cost of your visit.

Privacy Policy

The Neurology Clinic is fully compliant with the Health Insurance Portability And Accounting Act (HIPPA) of 1996.  You will be provided with a copy of our Notice of Privacy Practices to review prior to your appointment with Dr. Atkinson.  Please arrive early to allow time to review this information.
